It’s a painful mistake that hundreds of people make on an annual basis. The day before a family wedding, Anna Phillips was making plans to look her best in the formal photos. She bought standard over-the-counter whitening strips, popped them in her mouth and relaxed on her bed after dinner. Then—accidentally—she fell asleep.

“We see this kind of thing happen all the time,” says Floss Dentist Clark Steffens. “Sometimes the patients arrive in tears because of the severe pain.”

Annually, Americans spend $ 1.4 billion on over-the-counter whitening agents. Many, however, fail to follow the directions. Floss Dental has already seen approximately 10% more ‘bleach burn’ cases this compared to 2011

Anna woke up the next morning with throbbing teeth and gums; the gel had remained in place all night. “It was the worst oral pain I’ve ever experienced,” she says. “And I did it to myself!”

Most bleaching gels contain peroxide mixed with other chemicals. When the bleaching agent—even in its mildest concentration—is left in place for hours, the caustic chemicals can leach too deeply into tooth enamel, irritating nerve endings and gums. The longer the gel remains in place, the worse the irritation. “That’s the number one mistake made by people who try to do too much too quickly, or who accidentally leave the trays in too long,” says Dr. Steffens. “Leaving any bleaching agent in your mouth longer than 30 minutes is a major blunder.”

The second problem – less common – is attempting to apply a professional grade of bleach at home. “Only a trained dental hygienist or dentist should be applying the higher concentrations of oral bleach,” says Dr. Steffens. “We have to take real precautions and monitor the process in a clinical setting.” Generally speaking, the dental industry divides bleaching compounds into two categories: one for over-the-counter use, and the second (higher concentration) is only for use in dental practices, to be applied by technicians with the appropriate training. Most dentists regard anything with 20% or higher concentration of peroxide as a professional-grade bleaching agent.

The third blunder is consuming certain dark liquids or foods immediately after bleaching. The chemical compounds in most bleach temporarily open the pores in tooth enamel. So, if a dark food is consumed shortly after the bleach has been applied, it has a greater likelihood of staining teeth. Steffens says patients should completely refrain from dark food for the first twelve hours after bleaching, and limit dark foods as much as possible thereafter. “The major culprits are coffee, red wine, blueberries and chocolate,” says Steffens, “all the good stuff.”

As for whitening, the number one cosmetic procedure, parents should be aware of some concerns before seeking professional treatment for children and teens. Dr. Stanton provided the answers to common whitening questions asked by parents:

Q. What’s the average age when kids can whiten their teeth?

A. We look at each case individually. A major consideration is if the teen’s yellow tooth color is so socially unacceptable that it is affecting their self-esteem. We first educate parents about what causes teeth to yellow, then explain whitening benefits and risks. Generally, teens are safe to whiten once all of their permanent teeth have come in.

Q. What causes yellow teeth in kids?

A. The most common reason young people have yellow teeth, besides genetic factors, is the lack of good brushing and flossing habits. Another cause may be the antibiotics that many teens take for acne. Also if a teen drinks a lot of dark colored soda or pigmented foods such as tomato sauce, that can impact tooth color as well. The key is to brush and floss after each meal and visit the dentist at least twice a year for cleanings.

Q. Is there societal pressure to whiten at younger ages?

A. Sometimes, the whitening request is driven by the parent. However, girls have a higher awareness of themselves and how they project to others. Boys on the other hand, tend to not really notice (if at all) until they are in the work force or looking for that special someone to settle down with.

Q. Should teens whiten less often than adults?

A. Yes, because they typically respond faster and should only whiten as needed. In my practice, we use a lower percentage of whitening gel.

Q. Would either in-office whitening or home whitening kits be acceptable?

A. In-office works best since it allows the practitioner more control by doing the some of the whitening up front. The caution with young teeth is that the sensitivity after whitening can be greater, therefore it is important to manage the whitening process.

Q. Are there any other concerns?

A. When it comes to whitening a young person’s teeth, it’s okay for a parent or dentist to say “no” or “not right now.”

The Federal government defines an HSA as a tax-exempt trust or custodial account that a qualified individual sets up with an HSA trustee to pay or reimburse certain medical expenses that may be incurred. In short, HSAs are similar to a personal savings account, only the money must be used for health care expenses. Money in the HSA can be used to pay the HDHP deductible. Undispersed money remaining in the account earns interest.

Grant Born, principal at Healthcare Solutions Team, points out the advantages of HSAs to his customers, and lets them decide if it is the best option for them.

“Every individual is different, and so are his or her health insurance needs,” said Born. “One of the most overlooked and perhaps misunderstood health insurance options is the high deductible health plan (HDHP).”

This type of insurance has a higher annual deductible than most traditional co-pay plans. However, since insurance companies do not have to process and pay claims for routine or marginally expensive medical care, they can pass monthly savings on to customers. Plus, there is a maximum limit on the sum of the annual deductible and out-of-pocket medical expenses that must be paid for covered expenses. To finance HDHPs, many people turn to HSAs.

“With a typical co-pay insurance plan, the insured continues to pay for services, even though the amount they pay is only a percentage of the total cost,” said Born. “With the HSA or one-deductible program, the insured pays full price for services up front—but only until the calendar-year deductible is met. After that, the insurance provider typically pays 100 percent of the remaining covered expenses.”

A simple way to think about it is that with co-pay plans the deductible signals the beginning of payments, with HSA the deductible is the end.

Money deposited into an HSA can be used for a variety of medical expenses, including services that are not covered by insurance such as maternity costs; vision care or Lasik surgery; alternative care, acupuncture and chiropractic services; mental, physical and massage therapy; dental services such as braces and teeth whitening; and even long-term care. If someone loses his or her job, HSA funds can be used to pay for insurance premiums, as well.

“One of the major benefits of an HSA is that you control your own money,” said Born. “Many people think that if they don’t use all the funds in the account, they lose the money. But that’s not the case. Unused money in your HSA rolls over and continues to grow tax-deferred.”

Some insurers have their own banks and offer enrollment into an HSA at the same time as enrollment into the health insurance plan. Consumers do not need to set up an HSA account through an insurance carrier, though. They can use a qualified bank and invest money in mutual funds or other opportunities.

In addition to their flexibility, another major benefit of HSAs is that the money deposited is tax-free.

“It’s a triple threat tax deduction,” said Joe Eichman, principal at Healthcare Solutions Team. “The money goes in tax free, growth is tax deferred, and the money taken out of the account is tax free as long as you use the money toward medical expenses.”

Healthcare Solutions Team recommends clients open an HSA and put as much money into the account as they expect to incur in medical expenses. The account owner can move money from a savings account to an HSA. He or she also can transfer funds to an HSA one time only from an IRA without penalty. People 55 or older can do a catch-up contribution of up to $ 1,000. Also, an employer can contribute to an HSA and when doing so, the contribution is excluded from income.

Born and Eichman agree that an HSA is an attractive health insurance option for customers who want to know what their worst-case-scenario, total-out-of-pocket costs will be. HSAs also appeal to those who are generally healthy but want to ensure they can cover costs for potential future health care expenses.

“We tell our clients that the rule of insurance is that it needs to pay for the things you can’t afford to pay for yourself,” said Born. “Is an HSA right for you? Ask yourself, would you rather pay the insurance company more every month or pay the doctor more only when you need it?”
